- Choose your preferred language: English or Spanish.
- Enter the 14-digit survey code found on your receipt–you will see that the receipt is only valid for three days from your purchase.
- Choose the location of the restaurant you last went to and then click on “Yes”.
- You will have to enter the date and time written on the receipt and choose the service you used, such as Drive-Thru, Carry Out, or Dine-In.
- Then answer the questions about your order and experience with the restaurant.
- After you click on the button to continue the service you will be asked to rate your level of satisfaction.
- There are several questions regarding your visit and at the end of the survey, you receive a validation code that you need to write down on your receipt.

very disappointed. Monster taco was a monster shell the stuffed jalapeños was cold cheese not even completely melted. I ordered two tacos glad they screwed that up and only did one . don’t care for any free stuff only thing I found OK was my coke and I can get that at McDonald’s for $1

I have recently had the pleasure of ‘rediscovering’ Jack’s after 30 years of non-patronage. I grew up in Point Loma, Ca., (San Diego) not far from the original Jack’s at Voltaire St. and Sunset Cliffs Blvd., and ate there often after surfing in Ocean Beach, and all through High School. I’m not sure why I stopped, but for many years I did not go to Jack’s ….anywhere.
About a year ago I suddenly decided to give it a try in my new locale in the Monterey, Ca. area.
I tried the store on Munras Ave. in Monterey and the store on Fremont St., each a few times, and found that I was not convinced that my decision to return was a sound one. I won’t state the reasons, for they are not important here.
I then happened upon your store in the Del Rey Oaks area which according to my receipt is store #3478. I usually go there late and have truly enjoyed the food each time. The employees are friendly, helpful, positive, and always get my order right. This is important to any business. Further, I have found that this particular store is one of the cleanest and most orderly of any fast-food restaurant chain that I have ever been in. The items are always fresh, hot, and well made.
I am only writing this comment to commend your organization for having a refreshing and comfortable place for someone such as myself to drive the distance (I live in Pacific Grove, Ca. near Asilomar Beach) and know that what I buy there will be to my liking.

This Jack n the box has a new mgr. n it’s about time they hire polite employees.The cashier Michelle in the morning is polite,helpful friendly n always brightens my day with her smile n service. Also the cooks in back are friendly and make my food fresh n always gud.This makes my morning coffee n breakfast run an enjoyable expierence.
Hey Mr. Jack , Gabe here from El Paso Tx. Just wanted to say big fan! When I was in high school after a tough football practice, I’d go to one of your establishments and If I had enough lunch money saved from the week , I’d treat myself to a glorious sourdough burger. Years have passed and I still enjoy one. Do me a favor please! Bring back chilli cheese curly fries as an option and a burger from about ten years back. The bacon and cheese ciabatta burger!!!!!!! Jack in the box #177;Horrible. 4 visits in 6 months and only got the right order once. Staff is rude and not trained to deal with the public. No customer service skills. Will not be returning. Tried to complete the survey, but it kept skipping to the last page thinking me for my time, but I never got to complete the survey.
